  • Write your details and signature on the back of your ticket.
  • Print and complete the claim form and questionnaire. Ensure all your details are accurate, and take it to your appointment.
  • Take your photo ID and proof of your Social Security number. You may also take a W-2 or W-2G tax statement, a federal or state tax return, a 1099 form, or a Social Security card.
  • You must attend your claim center appointment alone unless you need help.

High-tier prizes exceeding $25,000 go to the Springfield Central Office. They need further payment processing and typically take 4–6 weeks for delivery.

Via Mail

Claiming any prize amount is possible via mail. Follow the steps below to ensure smooth processing.

  1. Print and complete the claim form and questionnaire. Make a copy and attach the necessary documents.
  2. Write your details on the back of your ticket, sign it, and make a copy.
  3. Write the correct address on an envelope. Illinois Lottery Claims Department, P.O. Box 19080, Springfield, Illinois 62794-9912.
  4. Write your return address on the back of the envelope. Put your completed claim form and ticket inside, and seal it properly.
  5. Use registered mail to send your claim via USPS.

Via e-Claim

You may claim your prizes via e-Claim if the amount is less than $10,000. Follow the steps below.

  1. Complete the e-Claim form, ensuring all your details are correct.
  2. You may need to upload a scan of your signed ticket. Ensure the image is clear.
  3. Follow all the on-screen prompts, then confirm and submit the e-Claim form.
  4. Wait until Illinois Lottery mails your winnings to you.
