Are you a group winner with a prize up to $999,999? A single player may collect the prize on behalf of the group. Just submit a completed claim form and the IRS Form 5754.

For group wins over $1 million, the claiming representative must complete CSL 0896, while the remaining group members complete CSL 0897.

When visiting a Lottery District Office, you don’t need an appointment. The offices are open Monday through Friday from 8 AM to 5 PM and operate on a first-come first-serve basis.

History of Largest California Lottery Prior Winners

Massive Mega Millions win

Daniel Beltran Eusebio started 2022 off with a bang when he won $2,390,663 on 5 January. His winning ticket matched five numbers but missed the Mega number.

Mom’s lucky numbers paid off

Each time that Laura Ivanoff bought a lottery ticket, she played her deceased mom’s lucky numbers. She knew they would have a big payout some day, and she was right. Playing SuperLotto Plus, she won a life changing $14 million.
